For all our visiting artists, please complete the Inventory Input form below. If you have any questions or concerns about your information, please contact our Marketing Manager at: [email protected] or call 719-685-1008.
  1. Use the print button to print a copy for your records. 
  2. Click the “Submit Form” button and the form with automatically emailed to you and the Commonwheel.  
  3. Once you hit “Submit Form” the document will disappear and you will not be able to print it.
